Objetivo
The PreDisc project aims to prevent disability in older people by promoting regular physical activity during and after periods of hospitalization. This is achieved by prescribing a personalized program during hospitalization and its continuation after discharge, thanks to its follow-up by Primary Care and facilitated the development of an innovative eHealth tool.
What was our contribution?
PreDisc focuses on promoting a model of active aging, especially in rural areas, through the development by the eVIDA team at the University of Deusto of a user-friendly technological tool with innovative guidelines to mitigate frailty. The objective is to promote equal access to healthcare by reaching rural areas and limiting the need for older people to move.
